Geriatric Care Management

Geriatric care management is still a relatively new field and, to our knowledge, is not regulated in any state. However, there is one professional association, the National Association of Professional Geriatric Care Managers (GCM), which has established professional standards of ethics and practice. Choices for Aging is a member of GCM.

Questions to Ask When Looking for a Geriatric Care Manager:

What are your professional credentials?

How long have you been providing care management services?
  Choices for Aging was incorporated in March, 1992.
Does your company also provide home care services?
  No.  Since we provide daily money management services, consultation and care management, we decided to not hire home care service workers and, hopefully, will avoid potential conflicts of interest.
What are your fees? (these should be provided in writing to the consumer or responsible party prior to services starting)
 
  • Assessment: $350 or $500, depending on case complexity
  • Care Management: $100/hour
  • Daily Money Management: $55/hour
  • Power-of-Attorney: $65/hour

(Choices for Aging also signs an Agreement for Services with each client.)

Are you licensed in your profession?

Are you available for emergencies?
  Yes, but we do not operate as a home health agency with availability 24 hours per day.
How do you communicate information to me?
  We consider communication vital and one of our company strengths. We give clients written assessments, monthly reports by email, fax or letter, regular telephone calls, cell phone accessibility and detailed monthly bills. Copies of bills and documents can be sent to third parties, upon approval from the client.
Can you provide me with references?
  Certainly, upon request.
